Hospitalization rates in dialysis patients have declined since 2007 but remain unacceptably high; 38% of dialysis patients are readmitted within 30 days of hospitalization.1 The transition of care from hospital to home is a time of increased risk for medication-related problems (MRPs). Programs to reduce MRPs have been shown to improve safety and effectiveness and reduce readmissions in the general population,2 but there is a paucity of data for dialysis patients.
