Erratum Regarding “Nephrologist-Facilitated Advance Care Planning for Hemodialysis Patients: A Quality Improvement Project” (Am J Kidney Dis. 2016;68[1]:103-109)
In the Original Investigation entitled “Nephrologist-Facilitated Advance Care Planning for Hemodialysis Patients: A Quality Improvement Project” that appeared in the July 2016 issue of AJKD (Amro et al, volume 68, issue 1, pages 103-109), there was an error in the reporting of the 12-month survival rate among patients whose nephrologists answered “no” versus “yes” to the surprise question. A total of 39 patients died, 22 in the former group (n=50) and 17 in the latter group (n=151).
