In the Original Investigation entitled “Nephrologist-Facilitated Advance Care Planning for Hemodialysis Patients: A Quality Improvement Project” that appeared in the July 2016 issue of AJKD (Amro et al, volume 68, issue 1, pages 103-109), there was an error in the reporting of the 12-month survival rate among patients whose nephrologists answered “no” versus “yes” to the surprise question.
