Idiopathic nodular sclerosis (INS) has a predilection for older white men. Patients usually present with reduced GFR and nephrotic-range proteinuria. Often patients have a clinical history of hypertension, tobacco use, hypercholesterolemia, and vascular disease. A clinical history of diabetes must be excluded.
