Inability to excrete salt with consequent expansion of extracellular fluid volume is a central feature of kidney failure. It contributes to many of the clinical problems seen in advanced chronic kidney disease, including hypertension, left ventricular hypertrophy and dilatation, and perhaps uremic inflammation and malnutrition.1,2 Correcting sodium overload and restoring sodium balance is therefore a primary aim of long-term dialysis therapy.
