Van Doorn et al1 present a valuable synthesis of sFlt-1/PlGF ratios and complement assays in pregnancy-associated thrombotic microangiopathy (TMA). I would like to highlight a conceptual issue raised by the case of patient COM005 that bears directly on clinical interpretation and bedside management.
