Toward Smarter Allocation by Rethinking Kidney Donor Profile Index

The Kidney Donor Risk Index (KDRI) and its percentile transformation, the Kidney Donor Profile Index (KDPI), are used to risk-stratify deceased donor organs as part of the kidney allocation policy. The original KDRI predicts allograft failure with a C-statistic of 0.61.1 This relatively low C-statistic means that KDRI only modestly discriminates between kidneys that will lead to allograft failure and those that will not. A C-statistic of 0.5 indicates random chance. Despite its limited predictive accuracy for allograft survival, the KDRI has a strong effect on the use of donor kidneys.
