Blood Pressure Lowering Agents for Kidney Transplant Recipients: Editorial Summary of a Cochrane Review
Hypertension is highly prevalent in kidney transplant recipients (1), and is associated with graft loss, adverse cardiovascular outcomes, and mortality (2, 3). Lowering blood pressure can reverse these complications (4). The 2009 KDIGO guideline on “Care of Transplant Recipients” reported no blood pressure lowering agent was contraindicated in kidney transplantation, and data from nontransplant patients with chronic kidney disease suggested angiotensin-converting enzyme (ACE) inhibitors or angiotensin II receptor blockers (ARB) could be considered as first-line therapy due to their potential beneficial effects on proteinuria (5, 6).
