Acute Kidney Injury in Care Transitions (ACT): A Randomized Controlled Feasibility Trial
Acute kidney injury (AKI) survivors experience gaps at care transitions. Inadequate monitoring of kidney function after discharge, exposure to avoidable nephrotoxic medications, and failure to start or resume nephroprotective drugs are associated with worse clinical outcomes, avoidable readmissions, and preventable adverse events in AKI survivors.1-3 Facilitating a safe care transition for AKI survivors requires a comprehensive model of care,4 not yet achieved with current care delivery models. To address gaps in care delivery, we developed and pilot-tested the AKI in Care Transitions (ACT) program.
