Nephrology providers have expertise in caring for patients with complex health problems related to chronic kidney disease (CKD), including concurrent CKD complications and commonly co-occurring chronic conditions such as diabetes and heart disease. Along with these problems, older adults with CKD often have other non-CKD-related chronic conditions and their associated medications, as well as functional limitations, cognitive impairment, and physical and mental health symptoms.1 The constellation of age-related problems and symptoms have been summarized in geriatric medicine as the Geriatric 5Ms.
