The educational tool and abstract to follow are intended for use by chronic kidney disease (CKD) and end-stage kidney disease (ESKD) patients, independent of kidney replacement modality, transplant, or conservative management. The abstract presents an overview of the handout intended to provide a tool for telehealth education to enhance preparation for and understanding and adoption of this mode of health care. The handout may be used in any medical management environment that intends to offer telehealth or is affiliated with service lines that incorporate telemedicine.
